This program serves all populations but specializes in Case Management and Support Services for individuals living with mental illness or experiencing some mental health  ( Social, Emotional, Behavioral) challenges. We provide individualized services (skill building) to ensure that participants are working toward self-sufficiency, while ensuring that they identify and achieve their person-centered goals.

 

Our Case Managers are Trained Qualified Mental Health Professionals that help link, arrange, coordinate, and advocate for each individual and family we serve. Our Goal is to teach the skills needed to promote mental wellness, how to live with mental illness, crisis stabilization, and create a team of natural and community supports to wrap around each individual we serve.
